Improving patient safety has become a major priority for the healthcare industry ever since the Institute of Medicine's groundbreaking 1999 report "To Err is Human". HCIF has been in the forefront of patient safety efforts in Southeastern Pennsylvania through the organization of large-scale regional initiatives to pool the resources and knowledge of hospitals and other providers.

This work is funded by the Partnership for Patient Care, a collaboration with Independence Blue Cross, which began in 2006 to accelerate the effective adoption of evidence-based clinical practices.  For more information on patient safety initiatives, see:
