The Union League of Philadelphia was the site of the May 26, 2010 Regional Symposium on Reducing Readmissions, which launched an 18-month collaborative, named the PAVE (Preventing Avoidable Episodes) Project,  to address issues related to key aspects of the discharge and care transitions processes.   The conference was attended by over 200 people, representing 40 hospitals and health systems and 20 non-acute providers and community organizations. Keynote speakers included national experts Eric Coleman, MD, MPH, University of Colorado; Brian Jack, MD, Boston University School of Medicine; and Mary Naylor, PhD, RN, University of Pennsylvania. The agenda included a series of breakout sessions on regional pilots and improvement projects already underway.
