Patricia Yurchick, RN, MBA, CPHQ, is the Senior Director of Quality Partnerships and Initiatives. In this role, she assumes responsibility for developing, implementing, and managing quality-focused programs funded by the Partnership for Patient Care, including leading the PAVE Project, an initiative to reduce readmissions in the region, which began in spring 2010, and working with nationally renowned consultants Healthcare Performance Improvement (HPI), LLC on the implementation of a high reliability safety culture with the region's hospitals. Additionally, she is serving as the Principal Investigator on a large, multi-year grant from the Pennsylvania Department of Health to improve health literacy in the cardiovascular population.
Patty comes to HCIF from the Pennsylvania Health Care Quality Alliance, where she was responsible for the development of its organizational strategy, including defining direction, reviewing revenue models, and expanding its constituency. Immediately prior to this, Patty was the Assistant Director in the Outcomes Management Department at MedStar Health, a multi-hospital system in the District of Columbia and Maryland, where she utilized various clinical informatics systems to identify and manage quality improvement initiatives at both the system and hospital levels. In prior positions, Patty worked as a healthcare consultant for CareScience, Inc. in Philadelphia (now part of Premier, Inc.) and Ernst & Young, LLP. Most notably, Patty was the Program Manager for the Cleveland Health Quality Choice Program, one of the first regional healthcare quality initiatives in the nation that served as a model for similar efforts elsewhere.
Patty is a registered nurse who also earned her BA from Baldwin-Wallace College and her MBA from Kent State University. She is a Certified Professional in Healthcare Quality (CPHQ) through the National Association for Healthcare Quality.
