This award process is one of the ways that HCIF promotes best practices in patient safety throughout the Delaware Valley and advances the goal of making the region the safest place to receive care.             <title>May 26th Regional Symposium on Reducing Readmissions: The PAVE Project</title>

            <description><![CDATA[The Union League of Philadelphia was the site of the May 26th Regional Symposium on Reducing Readmissions, which launched an 18-month collaborative named the PAVE (Preventing Avoidable Episodes) Project that will address issues related to key aspects of the discharge and care transitions processes.   The conference was attended by over 200 people, representing 40 hospitals and health systems and 20 non-acute providers and community organizations.  Keynote speakers included national experts Eric Coleman, MD, MPH; Brian Jack, MD; and Mary Naylor, PhD, RN.             <title>Annual Partnership for Patient Care Leadership Summit Held on March 16</title>

            <description><![CDATA[Teams from Southeastern Pennsylvania hospitals and health systems attended the March 16th Partnership for Patient Care Leadership Summit at The Down Town Club.  The agenda included an update on the multi-year, high-reliability performance initiative that began in 2009. Keynote speaker Marcia Delk, MD, MBA, Senior Vice President of Quality Affairs and Chief Quality Officer at WellStar Health System, presented on WellStar's patient safety culture journey with Healthcare Performance Improvement (HPI).  Several hospitals in this region then shared their experiences with their safety culture diagnostic assessment via a panel discussion.Storyboards from the top ten entries of the 2009 Delaware Valley Patient Safety Award were also on display.
