The PAVE Project was an 18-month regional collaborative aimed at improving care transitions and reducing readmissions that engaged healthcare providers and community-based service organizations across the continuum of care. HCIF is pleased to share some of the tools and data from this initiative: a Passport Series; aggregated monthly readmission rates; and a Transitions of Care Survey Summary.
